FEE FOR SERVICES
Dr. Ellen's practice accepts self-pay. Payment is due at the beginning of or prior to each session. You may use your preferred credit, debit, HSA, or FSA card for payment. Temporarily, check or cash is not accepted during the COVID-19 pandemic.
Intake assessment $250
Standard* therapy session (45-50 minutes) $180
*Alternate lengths for therapy sessions may be scheduled when appropriate per the client's treatment plan and in agreement between Dr. Ellen and the client. Such an agreement will require prorated session fees.
INSURANCE
If your insurance has an out-of-network benefit included in the coverage plan, you may choose to submit a reimbursement claim to your insurance provider. It is your responsibility to inquire your insurance provider regarding out-of-network reimbursement. You are encouraged to contact your insurance provider to ask about:
(1) coverage for out-of-network providers,
(2) limits on the number of visits in a calendar year,
(3) how much you will be reimbursed by the insurance company per visit.
It is your responsibility to understand your insurance coverage. Dr. Ellen may furnish you with a SuperBill should you wish to submit an out-of-network reimbursement claim to you insurance provider.
